I am on round 15 of the lace pattern.  What doe sthe abbreviation ko mean?

It means knit zero. knit none. Where as the large size has a knit 1 . I am knitting size large so I knit one. What I would do if I were knitting a S or M is work a YO and then start the knit 4 again.
